Screen potential eligible families for subsidized care
Enrollment and case management of eligible subsidy families for the CSPP/CCTR contract and private pay contracts
Intake/referral for all newly enrolled CSPP/CCTR families to Davis Street Support Services (i.e. Basic Needs, Primary Care Clinic, Behavioral Health, etc.)
Provide support, including assistance with department projects, data entry, eligibility list, follow up calls to providers and clients regarding incomplete information, etc.
Attendance at department meetings, monthly Agency-wide and offsite program-related meetings as required (CAPPA, CCDAA, EESD) Early Education and Support Division, Eligibility and Need workshops, etc.
Assist with maintaining site staff and family files at all sites
Assistance with department projects (document packets, database support, organizing records, etc.)
Provide resource and referrals as needed
Other tasks as assigned
REQUIREMENTS:
Experience in the areas of case management of families receiving subsidized childcare and social services. Experience in a non-profit sector is preferred
Knowledge of community support services available to subsidy families
General office knowledge, including operation of basic office equipment (fax machines, postage meters, copiers, etc.)
