Summary
Davis Street Community Center helps low-income families of San Leandro and surrounding communities by providing comprehensive services that assist clients with their financial, physical, and mental well-being. Davis Street programs include a Primary Care Clinic providing medical, dental and behavioral health services; food and clothing; subsidized child care; housing and utility assistance and referrals; childcare centers; and programs for individuals with development disabilities.
Responsibilities
· Function with autonomy and in collaboration with Providers.
· Provide health education to clinical staff
· Must be present at all times whenever medical services are provided
· Clinical duties including patient triage, data collection and recording, administration of vaccines and medications, medical records keeping, patient education, CLIA waived lab testing, labeling and transporting laboratory specimens
· Effective communication and teamwork with patients and providers
· Reviewing diagnostic test results, referring abnormal results to providers, and notifying patients of normal and abnormal findings
· Manage immunization programs including vaccine administration, data collection, reporting, ordering and transporting vaccines
· Management of Patient Assistance Program for medications including ordering prescribed medications and record keeping
· Coordinate with providers in the referral process, tracking referrals and referral follow-up
· Supervise, train and manage Medical Assistants
· Coordinate and manage data collection for QA/QI
· Other duties as assigned
Qualifications
· Current valid California RN License and BLS certification
· At least 2 years of experience in ambulatory or in hospital care setting as a Registered Nurse
· Bi-lingual English/Spanish preferred
· Skilled in Microsoft Office and EMR software
· Experience supervising clinical staff
