Job Description

The Medical Social Worker/Case Manager will work closely with the medical team to build capacity for HIV prevention activities, as well as assisting with other case management activities for the medical clinic as schedule permits.


  • Effective communication with patients, clinicians, and clinic staff members.
  • Participation in data collection and maintenance for quality assurance programs and grant deliverables.
  • Assist with care coordination and navigation of complex patients.
  • Assist patients with timely linkage to PrEP/PEP medications and use of Patient Assistance Program or other similar financial support options.
  • Provide education for clients receiving office-based testing for sexually transmitted infections.
  • Assist newly diagnosed HIV+ patients with linkage to specialty care.
  • Participate in community outreach events to promote general health and wellness as well as specific HIV prevention topics.
  • Accurate, thorough and professional documentation of all interactions with registered patients.
  • Maintain strict patient confidentiality.
  • Coordinate with Behavioral Health department to connect clients with therapy services as needed.
  • Counsel patients/clients on risk reduction strategies.
  • Patient/Client outreach as needed per discretion and as directed by clinical team.
  • Other duties as assigned.


  • Completion of an accredited Social Work program or related program of study (including Bachelor’s level)
  • Independent critical thinking and problem-solving skills
  • Current CPR/BLS certification (or completion within first 30 days of employment)
  • Skilled in Microsoft Office, data entry, and use of business email
  • Passion for care of medically underserved populations

$36-$39 per hour
