Job Description

The Behavioral Health Peer Navigator plays a key role in supporting clients with substance use disorders by leveraging their own lived experience with recovery. This position fosters trust, promotes engagement, and bridges gaps between clinical services and community resources. The Peer Navigator will support patients at various stages of recovery, help them access treatment and supportive services, and promote long-term wellness in alignment with Davis Street’s integrated care model.

Responsibilities

  • Build trusting relationships with SUD program participants through shared lived experience and empathy.
  • Provide peer-based support, recovery coaching, and mentorship to clients navigating treatment and recovery.
  • Assist clients with identifying and accessing community resources (housing, employment, recovery support, legal aid, etc.).
  • Collaborate with clinical teams to support care coordination, appointment adherence, and individualized care plans.
  • Conduct outreach and follow-up with clients who have disengaged or are at risk of treatment dropout.
  • Facilitate or co-facilitate recovery-oriented support groups and wellness activities.
  • Document client encounters, progress, and referrals in accordance with clinic policies and confidentiality standards.
  • Participate in multidisciplinary team meetings and training related to trauma-informed care, harm reduction, and cultural humility.

Requirements

  • Preferred: Bilingual (English/Spanish)
  • Preferred: Experience working in behavioral health, community health, or harm reduction settings.
  • Nice to have: Lived experience with substance use and sustained recovery (minimum 2 years preferred)
  • Nice to have: Certified Peer Support Specialist or Peer Recovery Coach (or ability to obtain within 6 months)
  • Ability to work compassionately with diverse populations, including individuals experiencing homelessness, co-occurring disorders, or systemic barriers.
  • Strong communication, advocacy, and interpersonal skills.
  • Ability to use computer and other electronic devices to utilize EHR system and companion software
  • Ability to organize, prioritize and handle multiple tasks with attention to detail
  • Excellent work habits: punctual, reliable, cooperative, team player
