Independent Living Coach (Adult Disability Program)

Job Description

The Independent Living Coach trains persons with developmental disabilities to live independently or support them in their efforts to live independently. Work closely with the client's family, case manager and other support team members to ensure quality services are provided.

Responsibilities

  • Build and maintain healthy relationships with participants on their caseload
  • Maintain appropriate boundaries with participants and follows all protocol and procedures
  • Partner with participants to develop person-centered Individualized Service Plans, provide skill training and support based on their goals
  • Develop rapport with case managers, care providers (if applicable), and other support team members, and maintain open lines of communication
  • Maintain participant records, evaluate instructional plan and monthly progress, and make revisions when necessary
  • Complete and submit paperwork in a timely and accurate manner
  • Participates in progress reports, case reviews and goal planning for each participant
  • Assist in developing each consumer’s awareness of his/her civil and legal rights
  • Other duties as assigned

Requirements

  • High school diploma or equivalent
  • Experience or interest in provide services to developmentally disabled adults
  • Valid California Driver’s license, reliable vehicle, and current auto insurance
  • CPR& First Aid certification
  • Must be reliable, punctual and able to work independently
  • Accurately deliver all necessary paperwork and documentation in a timely manner
  • Basic computer competency
  • Good verbal and written communication skills
  • Commitment to the mission and values of Davis Street
