Paying for Care

Davis Street Primary Care Clinic is a sanctuary facility.

We will treat you regardless of your ability to pay.

As a Federally Qualified Health Center, we accept Medicaid, Medi-Cal, Medicare, Denti-Cal, many private insurance plans (Alameda Alliance, Anthem Blue Cross, Blue Shield of CA, Health Pac) and self-pay patients on a sliding scale fee (based on ability to pay). Please remember to bring your insurance card with you to each appointment.

If you do not have health insurance, you can pay for your care on a sliding scale, based on your income. Payments, including insurance co-payments, can be made with cash, debit or credit card and are due on the day of your appointment.

If you have insurance coverage, please bring the following with you to each appointment:

  • Valid Photo ID
  • Copy of your insurance membership card​

If you do not have insurance coverage and need to self-pay, please bring the following documents with you to apply for a payment program, insurance coverage, or Medi-cal.

  • Valid Photo ID​
  • Birth Certificate or Passport or Social Security Card or Naturalization Card​
  • Income Verification: your previous two salary pay stubs or your previous two unemployment checks, or an award letter from Social Security, or your latest income tax return or W-2 Form.​

We are served by Stop #4 on the South Loop San Leandro LINKS , free shuttle route. If you drive, we have ample free onsite parking.
