• You can conveniently submit your child attendance records online.
• You can fill out a paper form and mail it to Davis Street at: 3081 Teagarden Street in San Leandro, CA 94577.
• You can drop off your attendance records in person at Davis Street.

Submitting your forms online allows you to record child attendance on your phone, tablet or desktop as often as you like. By using our online system on a daily basis, you’ll always be on top of your hours. Filling out a paper form, on the other hand, is less timely and often less accurate.

Records are due the first day of the month for the previous month. They are late after the 5th day of the month.

Davis Street processes your reimbursement on the 20th of every month. Depending on your preferences, funds will be deposited in your bank account, or we will mail you a check.
